Terms of Service

Welcome to The Yorkshire Wellness Hub, a service provided by DayNight Pharmacy. Please read these Terms of Service ("Terms", "ToS") carefully before using our weight management service.

By accessing or using our Service, you agree to be bound by these Terms. If you do not agree, do not use the Service.

1. Use of Service

  • You agree to use our Service only for lawful purposes and in accordance with all applicable laws and regulations.

  • You must provide accurate, current, and complete information during registration and consultation.

  • You agree not to misuse the Service by attempting to obtain medications through fraudulent means or by providing false information.

2. Eligibility

  • To use our Service, you must be aged 18–84 years and meet eligibility criteria as assessed by our clinicians.

  • You must provide access to your Summary Care Record (SCR) or equivalent verified medical information.

  • The Yorkshire Wellness Hub reserves the right to refuse service if eligibility or safety concerns arise.

3. Appointments and Consultations

  • Initial consultations must be conducted in person at our pharmacy branch.

  • Follow-up consultations may be remote or in person, but medication supply requires in-person visits.

  • Face-to-face monthly reviews are mandatory for continued medication supply.

  • You agree to attend all scheduled appointments or notify us at least 48 hours in advance if rescheduling or cancelling.

4. Payment and Deposits

  • Full payment is required before medication supply.

5. Medication Supply and Use

  • Medication will only be supplied following a clinical assessment and in accordance with PGD protocols.

  • You agree to follow all instructions provided by our healthcare professionals regarding medication use.

  • If you experience side effects or adverse reactions, you must contact us promptly.

6. Patient Responsibilities

  • You must provide truthful and complete health information.

  • You must comply with all appointment schedules and treatment plans.

  • If using a translator, you accept responsibility for ensuring accurate communication.

  • You must not attempt to obtain the same or similar medications from other providers without informing us.

7. Privacy and Data Protection

  • Your data will be handled in accordance with our Privacy Policy.

  • By using the Service, you consent to the collection and processing of your personal data as described.

8. Limitation of Liability

  • While we take every care in providing services, we cannot guarantee specific health outcomes.

  • We are not liable for adverse effects caused by medication, provided assessments and instructions were properly followed.

  • Our liability for negligence causing death or personal injury is not limited.

9. Changes to Terms

  • We may modify these Terms at any time.

  • Continued use after changes means you accept the new Terms.

10. Governing Law

  • These Terms are governed by the laws of England and Wales.

  • Any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales.
